The Emergence of Post-Acute Medicine

The charge toward fee-for-value from fee-for-service has accelerated the emergence of post-acute medicine, a medical model not bound by traditional delivery locales or roles.  For certain population segments, bringing medical care out of the acute and ambulatory settings and into the community is de rigueur to deliver the timely access and collaborative, team-based care required for success in a future defined by value.

Post-acute medicine delivers care in community-based and other non-traditional settings focusing on ensuring a “health home” is created for these chronically-ill, often frail patients. The locales include the patient’s place of residence, such as the home or assisted living facility, as well as SNFs and increasingly transitions clinics. The focus is not merely primary care, but importantly meeting the non-medical needs of the patient in their current setting to ensure quality outcomes.

Health Dimensions Group National Summit | Recap

The 5th Annual Health Dimensions Group National Summit was a blast. We cannot believe that only two weeks ago we were in sunny Orlando, Florida.

The conversations that were buzzing around at the National Summit created an environment that is unique to most other professional settings. The value from these industry experts gathering in one place comes from utilizing combined past experiences to predict future industry trends. Betty Ice, Founder of the National Summit and Vice President of Marketing and Communications at HDG explained the initial idea behind the conference, “The National Summit originated with the idea of gathering healthcare leaders in an environment to participate in leading edge conversations. The National Summit allows for individual knowledge and experiences to be shared and valuable, long-term connections to be made.”

According to first-time attendee William W. Hartung, CPA at Health Care Data Consulting LLC, this was one of the most advanced conferences he has ever been to. “The discussions on the transformation in post acute and acute care have provided ideas and actionable information that I will use in my own local market so our provider setting can succeed in our service area,” William stated.

Not only did attendees walk away with actionable insights, they also made valuable connections with professionals from across the country at all different levels. These new relationships provide great potential for collaboration and partnerships that would not have otherwise been possible without the Summit to bring them together.

Health Dimensions Group National Summit | Day 2

Day 2 of the National Summit started bright and early yesterday morning. We spent breakfast eating and mingling with our wonderful sponsors from near and far.

Right after breakfast attendees had the option to choose between two different session tracks: “Alternative payment Arrangements: Learning how to Ride the New Wave” and “The Doctor is Really In: Expanding Primary Care in the Senior Housing and Elder Care Environment.”  Although picking between these two sessions was a tough choice, attendees found value in both sessions that they would be able to apply to their individual organizations.

After a quick break to check out the sponsor booths, attendees were faced with another difficult choice between two dynamic concurrent sessions. One option was “Alternative Payment Arrangements: Building Win-Win Relationships with Managed Care Organization, Bending the Curve and Sharing the Gain” and the other option was ”The Technology Value Proposition: Integrating Tools, Systems and People to Improve Elder Care”

We enjoyed a delectable lunch and some free time, and then wrapped up our day with a workshop intensive on bundled payment in the industry. The panel, moderated by Jade Gong, senior advisor at Health Dimensions Group, really got into the finer details of redesigning care delivery processes and increasing the levels of patient activation.

Day 2 made it easy to understand why the National Summit has become the event to mark on your calendar year after year. Rodney McBride, vice president of health and community services at John Knox Village, explained to us why this is his fourth year back at the National Summit. “There is a high level conversation that takes place at this conference that does not occur anywhere else” explains McBride. “The integration of post acute and hospital information is very forward looking. I’m excited to implement what I’m learning here from an operational and strategic standpoint.”

After a busy day full of new connections and new information, attendees dispersed to explore the abundance of activities that Orlando, Florida has to offer before the final day of the National Summit. We can’t believe it’s almost over, but mark your calendars! The 2016 Summit dates have officially been announced. We hope you can join us in Las Vegas on February 22-24, 2016.
